Heal skills generally refer to the 6th slotted skill. Obviously there are sub-effects for certain skills that also heal, but they are not able to activate most traits that specify "heal skill".
It is the same for the opposite direction too, you may have a trait that says when you use a secondary skill X happens. An example of this is the mesmer trait that applies to mantras, each is castable 3 times before needing to be focused, and only effects secondary skills, and not the heal skill since the heal skill is not considered a secondary skill.
However it should be noted that boons do not have such a limitation. Thus if you gain regeneration through a skill, or from a rune bonus (like the 6 slot bonus for Dwayna), you'd be able to get any secondary effects from traits or skills that use/effect such a boon.
